Intermediate Ladies Line Dance Workshop

As one of the conditions of my participation, I specifically release Heart & Soul Line Dance, its instructors, assistants, and affiliates, from any claims or liabilities of any kind whatsoever arising from or related to my participation in the Heart & Soul Line Dance Workshops.  I understand that any physical activity involves risk or injury and that one should consult a doctor before beginning any new physical activity.   My voluntary participation and signature denote full assumption of that risk and responsibility of any potential injuries.

I agree to grant to Heart & Soul Line Dance and its authorized representatives permission to record on photography film and/or video images my participation in the Heart & Soul Line Dance workshops.  I further agree that any or all of the material photographed may be used in any form as part of any current or future publications brochures, fliers, social media use or other materials associated with Heart & Soul Line Dance and that such use shall be without payment of fees, royalties special credit or other compensation.
